Welcome to online clergy license renewal! Read this entire page carefully before you begin.The following series of forms will guide you through the process of filing your 2012 Clergy Licensure information with the Office of Formation and Leadership Development.
This process will include five steps:
- Step 1 – The Licensure Application
- Step 2 – Education & Continuing Education Filing
- Step 3 – Code of Conduct Signature
- Step 4 – Clergy License Payment
- Step 5 – Download, Print & Complete the Certifying Signature Form
IMPORTANT NOTES:
Completion of this form requires the return of your Certifying Signature Form and your licensure payment. Once all information is received and verified your License to Practice will be mailed to you.
You must verify your Continuing Education with a certificate, transcript or any other documentation indicating completion of the 9 hours. (These may be uploaded via the Education & Continuing Education online form.)
Chaplains must have a separate application to serve as MCC Chaplain on file with the Office of Formation and Leadership Development. To download the application, click here.
ITEMS TO HAVE PRIOR TO BEGINNING THIS ONLINE PROCESS:
- Your full personal information, including full addresses, birth date, social security number (for US citizens).
- Information about your Ordination and MCC involvement, including: Year of Ordination, Local Church Affiliation, Hourly Investment in Ministry
- Information on your Triple A Ministry, including: Name, Location, Length of Ministry
- Education and Continuing Education Information, including: School and/or Workshop Name, Dates of Education, Presenter Name(s), Electronic Copies of any Certificates (if available)
